How to use adrenaline auto-injectors like EpiPen®, Jext® or Emerade®

There are three main types of adrenaline auto-injector which are prescribed in the UK; EpiPen®, Jext® and Emerade®. Emerade is the most recent single use adrenalin auto-injector to become available in the UK. All are disposable, pre-filled automatic injection devices.

You may also have heard of Anapen®. Whilst Anapen® is in use in Ireland, the US and Europe it is not currently passed for use in the UK.

EpiPen®

EipPen® is available in the following doses, the dose is decided by your doctor:

  • 0.3mg (yellow label) - For adults and children weighing over 25kg
  • 0.15 mg Junior - For children weighing between 7.5 and 25kg

Emerade®

  • 150 micrograms
  • 300 micrograms
  • 500 micrograms

Use in Children:

  • Emerade 500 micrograms is not recommended for use in children.
  • Children below 15 kg bodyweight: A dosage below 150 micrograms cannot be administered with sufficient accuracy in children weighing less than 15 kg and use is therefore not recommended unless during a life-threatening situation and under medical advice.
  • Children between 15 kg and 30 kg bodyweight: The usual dose is 150 micrograms.
  • Children over 30 kg bodyweight: The usual dose is 300 micrograms.
  • Adolescent patients over 30 kg bodyweight: The dosage recommendations for adult patients should be followed.

Use in Adults

  • The recommended dose is 300 micrograms for individuals under 60 kg bodyweight. The recommended dose is 300 to 500 micrograms for individuals over 60 kg bodyweight, depending on clinical judgement.
  • An initial dose should be administered as soon as symptoms of anaphylaxis are recognised. In the absence of clinical improvement or if deterioration occurs, a second injection with an additional Emerade may be administered 5 – 15 minutes after the first injection. It is recommended that the patients are prescribed two Emerade pens which they should carry at all times.

The Emerade website has useful information and resources for patients as well as their famiy and friends. There is an expiry reminder service where you can register your pen tso that you can be sure to re-order one before it goes out of date. There is also a free mobile app which includes instructions for Emerade as well as symptoms of anaphylaxis.

Did you know that you can carry Emerade in your hand luggage? You may need your doctor to sign a travel certificate. Emerade have an online form to print a travel certificate for your doctor to sign.

A MUST HAVE for anyone who carries a JEXT pen, the app allows you to show others your around exactly how to use your pen (if it’s necessary) by a really quick short video that can be life saving! You can also register your pens expiration date and the app will give you a reminder to order a new one via your gp. You can receive a free training pen (which to me is the best thing ever) I can just use the training pen to show anyone how to use my adrenaline pen on me if my anaphylaxis strikes, it’s made a huge difference in making friends more comfortable in knowing what to do, and also other family members. I think the training pens should be in every work place they are genuinely the difference between life and death, someone who knows what an adrenaline pen is and someone who doesn’t is a huge difference. Anyway 5 star app haha

How to use Jext ®

Grasp the Jext ® in your dominant hand (the one you use to write with) with your thumb closest to the yellow cap.

Pull off the yellow cap with your other hand.

Place the black injector tip against your outer thigh, holding the injector at a right angle (approx. 90°) to the thigh.

Push the black tip firmly into your outer thigh until you hear a ‘click’ confirming the injection has started, then keep it pushed in. Hold the injector firmly in place against the thigh for 10 seconds (a slow count to 10) then remove. The black tip will extend automatically and hide the needle.

Massage the injection area for 10 seconds. Seek immediate medical help.

Extra steps to take

Call 999 in the UK, or 112 for Ireland, ask for an ambulance and say ‘anaphylaxis’. If you are unable to make the call, get someone else to call for you.

Use a second Jext® after 5-15 minutes if the symptoms do not improve. Always carry an extra Jext® with you.

Booking Your Flight

When booking your flight there are a number of steps you can take to ensure your journey is as smooth and safe as possible, including:

Research the different airlines’ allergy policies – are there allergy-free meals available?

Pick your time of travel. Aeroplanes are usually cleaned overnight, lowering the chance of contaminated surfaces. If you have an allergy to nuts , flying first thing in the morning is probably a good idea. But if you have an egg or milk allergy , a flight outside of breakfast hours may be a more suitable option.

A child with an allergy should never travel unaccompanied by an adult who can assume their care.

If you are worried, discuss your proposed flight with your GP or specialist.

After You've Booked Your Flight

Check expiry dates on your or your child’s adrenaline auto-injectors and any other medication before you travel. Give yourself plenty of time to get a new prescription if your medication is due to become out of date while you’re away.

If you have been prescribed adrenaline auto-injectors, practise regularly with a trainer device so you are confident in how to use it. Preparation

Carry at least two adrenaline auto-injectors when travelling. Depending on the location of your travels, you may require more than two devices, especially if you’re going somewhere quite remote.

Documentation

You may be asked for documentation to travel on a plane with your adrenaline auto-injectors. The three adrenaline auto-injector manufacturers have Travel Certificates available on their websites at – www.emerade-bausch.co.uk – www.epipen.co.uk – www.jext.co.uk

Allergy action plan

Complete and take an allergy action plan with you, in case of emergency.

Make sure your child’s allergy action plan is up-to-date, and take it with you. This needs to be completed by the child’s health care professional.

Labelling laws

Check the food labelling laws in the country you’re visiting. Labelling laws will vary around the world. Labelling laws across the EU are consistent with the UK.  Australia, New Zealand and the USA have their own food labelling regulations. You can find out more information on international food allergen labelling regulations here.

Emergency contacts

Find out where the nearest hospital is and how to contact the emergency services in the country you are visiting.

Fill in your medical ID on your mobile phone, which can usually be found on the lock screen on the emergency call screen. You can tap in a contact number and the details of your allergies, reactions and medication.If you are travelling abroad, you could write this information in the local language (as well as in English).

Translation cards

Organise translation cards in the language/s for the country/countries you are visiting. Write down a list of translations of what you are allergic to and how to ask anyone about it.

Before Leaving Home

  • Check you have all your prescribed medication. Carry this in your hand luggage, not your suitcase.
  • Wear medical identification (e.g. a medical alert bracelet or necklace) indicating your allergies.
  • Ideally pack your own food for the flight, instead of eating airline food. However, do check to see if there are any restrictions as to which types of food you are allowed to bring through airport security.
  • Leave early for the airport to allow yourself plenty of time to reconfirm any requests for specific seating, early boarding etc.

On the Flight

  • Keep your adrenaline auto-injectors on you; do not put them in your packed luggage or store in the overhead locker. Let others you are travelling with know about your allergies and where your adrenaline auto-injectors are. If travelling alone, inform the flight attendants.
  • Use wet wipes to wipe down the seat and tray table to help prevent contact reactions or inadvertent skin contact with food particles or spills. Eating food from a contaminated surface area could lead to accidental ingestion of allergens.
  • Avoid using the airline’s pillows and blankets, as they are often not washed between flights, only rewrapped.
  • Never take any unnecessary risks, especially when in the air away from access to medical help.
  • If you have an allergic reaction, use your adrenaline auto-injectors as soon as symptoms start, and inform the flight crew immediately. Also ask if there is a doctor (or medical professional) on board to assist you. If possible, go to an area of the plane to lie down, but avoid unnecessary physical activity.

When Abroad

Eating out should be a fun and enjoyable part of your time away before heading out. It would be a good idea to check the following details with those you are with.

Make sure they are aware of your or your child’s allergy (or allergies) and that you are carrying adrenaline auto-injectors (AAIs). If the risks are high on a particular occasion, it may be best not to eat anything or it may be best to ask your accommodation manager if they can prepare a packed lunch free from your allergens.

If a reaction does occur, follow emergency instructions and use your adrenaline first.

Your Medication

Make sure you have your medication on you at all times . If you have been prescribed adrenaline auto-injectors, always carry two .

When you have arrived at your destination there are various ways that you can carry your adrenaline auto-injectors discreetly. However, make sure you let whoever you are travelling with know where they are and how to use them if a reaction occurs. Make sure those you are travelling with are aware of the symptoms of anaphylaxis.

Storing Your Adrenaline Auto-Injector

Do not leave adrenaline pens in direct sunlight. Excessive heat can cause the adrenaline to break down, meaning it might not be as effective when you need it.

Do not expose your adrenaline auto-injectors to cold temperatures. For EpiPen devices, there is additional instruction to not refrigerate the device.

Bags are available to maintain medication at the correct temperature. Please refer to the patient information leaflet accompanying your device.

Never leave your AAIs in a cloakroom, on a coach or other place where bags etc are stored as you need immediate access to your medicine at all times.

How to use video

It’s important that you learn how to use Jext ® because you must use it as soon as you have any symptoms of an anaphylactic reaction.

Administering Jext ® is not hard, but it may not seem easy during an anaphylactic reaction because you might feel a bit frightened. However, if you learn properly how to use Jext ® , this won’t be a problem and you will feel better about using your Jext ® .

By practising with a Jext ® Trainer whilst watching the demonstration video, you and your family and friends can feel confident in using a real Jext ® in an emergency.

Your Jext® has an expiry date and you can register with the free ALK Expiry Alert Service to receive alerts warning you when your Jext® is due to expire and needs to be replaced by your GP.

We will send through notifications by either SMS or email, or both, notifying you when your Jext® is due to expire. When your Jext® expires, visit your GP to dispose of the expired Jext® and collect a replacement.

It is important that you are fully aware of your allergy. Spend time learning about your allergy and discussing ways in which you can minimise risks when you are not at home. If your Jext ® is kept at school, make sure the expiry date is registered with the expiry alert service and you know what to do in an emergency with the person in charge.

Carry two Jext ® pens

It is recommended you carry two Jext ® pens with you at all times. Early use of Jext ® improves the outcome and recovery from anaphylaxis, but if symptoms don’t improve or the symptoms get worse, a second Jext ® can be used after about 5-15 minutes.

The Moscow Metro Museum of Art: 10 Must-See Stations

There are few times one can claim having been on the subway all afternoon and loving it, but the Moscow Metro provides just that opportunity.  While many cities boast famous public transport systems—New York’s subway, London’s underground, San Salvador’s chicken buses—few warrant hours of exploration.  Moscow is different: Take one ride on the Metro, and you’ll find out that this network of railways can be so much more than point A to B drudgery.

The Metro began operating in 1935 with just thirteen stations, covering less than seven miles, but it has since grown into the world’s third busiest transit system ( Tokyo is first ), spanning about 200 miles and offering over 180 stops along the way.  The construction of the Metro began under Joseph Stalin’s command, and being one of the USSR’s most ambitious building projects, the iron-fisted leader instructed designers to create a place full of svet (radiance) and svetloe budushchee (a radiant future), a palace for the people and a tribute to the Mother nation.

Consequently, the Metro is among the most memorable attractions in Moscow.  The stations provide a unique collection of public art, comparable to anything the city’s galleries have to offer and providing a sense of the Soviet era, which is absent from the State National History Museum.  Even better, touring the Metro delivers palpable, experiential moments, which many of us don’t get standing in front of painting or a case of coins.

Though tours are available , discovering the Moscow Metro on your own provides a much more comprehensive, truer experience, something much less sterile than following a guide.  What better place is there to see the “real” Moscow than on mass transit: A few hours will expose you to characters and caricatures you’ll be hard-pressed to find dining near the Bolshoi Theater.  You become part of the attraction, hear it in the screech of the train, feel it as hurried commuters brush by: The Metro sucks you beneath the city and churns you into the mix.

With the recommendations of our born-and-bred Muscovite students, my wife Emma and I have just taken a self-guided tour of what some locals consider the top ten stations of the Moscow Metro. What most satisfied me about our Metro tour was the sense of adventure .  I loved following our route on the maps of the wagon walls as we circled the city, plotting out the course to the subsequent stops; having the weird sensation of being underground for nearly four hours; and discovering the next cavern of treasures, playing Indiana Jones for the afternoon, piecing together fragments of Russia’s mysterious history.  It’s the ultimate interactive museum.

Top Ten Stations (In order of appearance)

Kievskaya station.

Kievskaya Station went public in March of 1937, the rails between it and Park Kultury Station being the first to cross the Moscow River.  Kievskaya is full of mosaics depicting aristocratic scenes of Russian life, with great cameo appearances by Lenin, Trotsky, and Stalin.  Each work has a Cyrillic title/explanation etched in the marble beneath it; however, if your Russian is rusty, you can just appreciate seeing familiar revolutionary dates like 1905 ( the Russian Revolution ) and 1917 ( the October Revolution ).

Mayakovskaya Station

Mayakovskaya Station ranks in my top three most notable Metro stations. Mayakovskaya just feels right, done Art Deco but no sense of gaudiness or pretention.  The arches are adorned with rounded chrome piping and create feeling of being in a jukebox, but the roof’s expansive mosaics of the sky are the real showstopper.  Subjects cleverly range from looking up at a high jumper, workers atop a building, spires of Orthodox cathedrals, to nimble aircraft humming by, a fleet of prop planes spelling out CCCP in the bluest of skies.

Novoslobodskaya Station

Novoslobodskaya is the Metro’s unique stained glass station.  Each column has its own distinctive panels of colorful glass, most of them with a floral theme, some of them capturing the odd sailor, musician, artist, gardener, or stenographer in action.  The glass is framed in Art Deco metalwork, and there is the lovely aspect of discovering panels in the less frequented haunches of the hall (on the trackside, between the incoming staircases).  Novosblod is, I’ve been told, the favorite amongst out-of-town visitors.

Komsomolskaya Station

Komsomolskaya Station is one of palatial grandeur.  It seems both magnificent and obligatory, like the presidential palace of a colonial city.  The yellow ceiling has leafy, white concrete garland and a series of golden military mosaics accenting the tile mosaics of glorified Russian life.  Switching lines here, the hallway has an Alice-in-Wonderland feel, impossibly long with decorative tile walls, culminating in a very old station left in a remarkable state of disrepair, offering a really tangible glimpse behind the palace walls.

Dostoevskaya Station

Dostoevskaya is a tribute to the late, great hero of Russian literature .  The station at first glance seems bare and unimpressive, a stark marble platform without a whiff of reassembled chips of tile.  However, two columns have eerie stone inlay collages of scenes from Dostoevsky’s work, including The Idiot , The Brothers Karamazov , and Crime and Punishment.   Then, standing at the center of the platform, the marble creates a kaleidoscope of reflections.  At the entrance, there is a large, inlay portrait of the author.

Chkalovskaya Station

Chkalovskaya does space Art Deco style (yet again).  Chrome borders all.  Passageways with curvy overhangs create the illusion of walking through the belly of a chic, new-age spacecraft.  There are two (kos)mosaics, one at each end, with planetary subjects.  Transferring here brings you above ground, where some rather elaborate metalwork is on display.  By name similarity only, I’d expected Komsolskaya Station to deliver some kosmonaut décor; instead, it was Chkalovskaya that took us up to the space station.

Elektrozavodskaya Station

Elektrozavodskaya is full of marble reliefs of workers, men and women, laboring through the different stages of industry.  The superhuman figures are round with muscles, Hollywood fit, and seemingly undeterred by each Herculean task they respectively perform.  The station is chocked with brass, from hammer and sickle light fixtures to beautiful, angular framework up the innards of the columns.  The station’s art pieces are less clever or extravagant than others, but identifying the different stages of industry is entertaining.

Baumanskaya Statio

Baumanskaya Station is the only stop that wasn’t suggested by the students.  Pulling in, the network of statues was just too enticing: Out of half-circle depressions in the platform’s columns, the USSR’s proud and powerful labor force again flaunts its success.  Pilots, blacksmiths, politicians, and artists have all congregated, posing amongst more Art Deco framing.  At the far end, a massive Soviet flag dons the face of Lenin and banners for ’05, ’17, and ‘45.  Standing in front of the flag, you can play with the echoing roof.

Ploshchad Revolutsii Station

Novokuznetskaya Station

Novokuznetskaya Station finishes off this tour, more or less, where it started: beautiful mosaics.  This station recalls the skyward-facing pieces from Mayakovskaya (Station #2), only with a little larger pictures in a more cramped, very trafficked area.  Due to a line of street lamps in the center of the platform, it has the atmosphere of a bustling market.  The more inventive sky scenes include a man on a ladder, women picking fruit, and a tank-dozer being craned in.  The station’s also has a handsome black-and-white stone mural.

Here is a map and a brief description of our route:

Start at (1)Kievskaya on the “ring line” (look for the squares at the bottom of the platform signs to help you navigate—the ring line is #5, brown line) and go north to Belorusskaya, make a quick switch to the Dark Green/#2 line, and go south one stop to (2)Mayakovskaya.  Backtrack to the ring line—Brown/#5—and continue north, getting off at (3)Novosblodskaya and (4)Komsolskaya.  At Komsolskaya Station, transfer to the Red/#1 line, go south for two stops to Chistye Prudy, and get on the Light Green/#10 line going north.  Take a look at (5)Dostoevskaya Station on the northern segment of Light Green/#10 line then change directions and head south to (6)Chkalovskaya, which offers a transfer to the Dark Blue/#3 line, going west, away from the city center.  Have a look (7)Elektroskaya Station before backtracking into the center of Moscow, stopping off at (8)Baumskaya, getting off the Dark Blue/#3 line at (9)Ploschad Revolyutsii.  Change to the Dark Green/#2 line and go south one stop to see (10)Novokuznetskaya Station.
