Cruise Ship Jobs for Doctors and Nurses

Even prior to the COVID-19 pandemic, there was always at least one doctor and nurse on each cruise ship. With new health regulations, most cruise lines are increasing the number of medical staff they have onboard. For example, the Royal Caribbean Group’s Healthy Sail Panel recommendations states that,

“As a part of augmenting onboard medical capacity to ensure preparedness for potential COVID-19 cases, cruise operators should increase their existing ratios of medical personnel to guests and crew.”

Cruise lines have begun sailing worldwide, and as capacity of their ships increase and more ships come back into service, the need for more doctors and nurses is required. Additionally, a Public Health Officer is new cruise ship job that has been added by many of the cruise lines.

Cruise Ship Medical Exam

After a successful interview, your cruise recruiter will inform you that you need to get a medical exam done before you are officially hired. Learn the reasons why it is required, what it involves, what you will need to disclose and which doctors are authorized to conduct a Seafarer medical exam.

Why You Need a Medical Exam to Work on a Cruise Ship

Over 70 years ago, the International Labour Organisation (ILO) decided that uniform regulations concerning the medical examination of seafarers were needed. Convention No. 73 declares that a person employed on a ship must have “…a certificate attesting to his fitness for the work for which he is to be employed at sea signed by a medical practitioner.”

The ILO also states that the crew member be “…in good health and not likely to endanger the health of other persons on board.” Consider that crew members live close to each other on board and may have contact with passengers. Therefore if a crew member had a contagious disease it would endanger the health of their fellow crew members along with passengers.

All cruise ship crew must be medically, physically and mentally fit for the job that they are employed to do. This includes being fit to perform emergency duties if required and able to deal effectively with isolation from friends and family. The doctor must attest to the overall fitness of the potential employee and sign a certificate saying so.

Overview of Medical Exam for Crew Members

There are two main types of medical exams for working on cruise ships. The pre-sea exam is very stringent as it may prevent a person from starting a career at sea if they don’t meet the requirements. Alternatively, the periodic exam certifies that the seafarer continues to be fit for sea service taking into consideration the intended cruise job that the person will be doing on board.

The actual name of the medical exam depends on the country you are taking the exam. In Canada, for example, it’s called a Mariner’s medical certificate.  In the United States it’s called a CG-719K. You can find the Merchant Mariner Physical Examination instruction guide located here   (pdf) . The UK and many other countries call the exam an ENG1.

These medical exams are conducted to identify medical conditions, examine previous medical history, detail any injuries, identify substance abuse and overall make an assessment that the crew member is fit to work on a cruise ship. Doctors will also assess a crew member’s mobility, balance, strength, and endurance. There must not be any physical disability issues.

Cruise Industry Approved Doctors

It’s important to know that not all doctors are authorized to conduct a seafarer medical examination. Your recruiter will give you a list of doctors available in your area. They may also send you specific forms that need to be completed by you and the doctor.  Here are sources for cruise industry approved doctors in Canada and the United Kingdom:

What to Expect During a Cruise Ship Jobs Medical

You must make an appointment with an approved doctor. Although you may not be informed by the doctor’s office, there are some items that you need to bring with you. Here is a brief list.

  • Glasses or contacts (don’t wear contacts to   the doctor’s appointment since you will be required to remove them)
  • Copy of vaccinations (World Health Organization – International Travel and Health )
  • List of medications that you take

The exam typically starts with the crew member completing a health questionnaire, detailing medical history, ailments, diseases, vaccinations, surgical operations, injuries and current medication. It includes personal information such as age, height and weight.

Next, the crew member is weighed, measured and given a urine test. Some doctors may also conduct blood tests including a blood glucose test. Hearing and vision tests are also given along with testing blood pressure and pulse rate. A chest x-ray may also be taken.

The doctor conducts a physical examination that includes asking questions based on the answers provided in the questionnaire. When you are given the medical certificate will depend in part on how long it takes to get the results of your blood and urine tests. The certificate is valid for a maximum of two years and typically costs from US$50 up to US$250, depending in which country the exam takes place.

Questions to Expect During Your Seafarer Medical Exam

The health questionnaire can be intimidating. It is usually a couple pages long and there may be questions that challenge your willingness to be perfectly honest. After all, you really want to work on a cruise ship—so you don’t want to fail the medical exam! The best advice, however, is that you should be completely honest. So that there are no unexpected surprises, here are typical questions that are on a pre-employment medical exam for a cruise ship job.

  • Do you wear glasses or contacts?
  • How much do you drink per day/per week?
  • How much do you smoke per day/per week?
  • What medication do you take?
  • How much exercise do you do per day/per week?
  • Have you suffered from a major depression?
  • Have you ever had substance abuse issues in your life?
  • Do you have diabetes?
  • Do you have asthma?
  • Have you ever had a seizure?
  • When was your last seizure?
  • Do you suffer from migraines?
  • Do you have any allergies?
  • Do you have any skin infections?
  • Do you suffer from varicose veins?
  • Do you have any STIs (STDs)?
  • Do you have AIDS?
  • Do you get nose bleeds?
  • Do you suffer from seasickness?
  • What vaccinations have you had?

The doctor will address your Body Mass Index (BMI) based on your height and weight, paying attention for symptoms of diabetes. A person with a high BMI may be unfit because they may not be able to perform their emergency duties.

Hiring Process after the Seafarer Medical Exam is Complete

Once you have been given your medical certificate, you need to send a copy to your recruiter and keep the original for yourself. You will need to bring the original on the ship when you join, but you should also keep another copy for yourself. If you have been denied the medical certificate, then you won’t be allowed to join a ship. You do have the right to appeal, though.

MSC Cruises offers a gluten-friendly selection gluten-free menus (breakfast, lunch and dinner) in the main restaurants on most of its ships. On MSC Armonia, MSC Sinfonia, MSC Opera and MSC Lirica guests can find only a limited selection of pre-packaged gluten-free snacks such as biscuits, croissants, sponge cakes and muffins. Guests with food intolerances and/or allergies must inform MSC Cruises of their needs via the Special Needs Form and, once on board, confirm their requirements at Reception-Guest Service and/or with the Maître d’hôtel. Kosher meals (pre-packaged breakfast, lunch and dinner) may be available on board for an additional charge of €25/$30 pp/day. Kosher meals can be consumed in the main restaurants and must be booked at least 30 days before ship’s departure. Halal products (beef, lamb and poultry) may be available on board following former request of the guest at least two months before departure. Vegetarian and Vegan -friendly dishes are incorporated into our main restaurant menus and buffet restaurant culinary offer. We can accommodate most special dietary requirements in our main restaurants where there is a greater degree of control over the production and service of special meals. We cannot guarantee no cross-contamination at the buffet.

Do on board staff receive special training in providing assistance to passengers with disabilities or reduced mobility?

Can i undergo dialysis treatment on board, are there excursions accessible to passengers with disabilities or reduced mobility, what is the deadline for notifying msc cruises of my special needs, can i travel with my guide dog, are there signs for passengers with visual disabilities, who will handle my special needs request and how will it be treated, how soon will i receive a reply to my special needs request, could msc cruises refuse my booking or deny me boarding, can a passenger with reduced mobility travel alone, which areas of the ship are accessible to passengers with disabilities or reduced mobility, are there cabins for passengers with disabilities or reduced mobility, can i bring my mobility scooter/wheelchair on board.

Due to safety considerations, wheelchairs and scooters cannot be stored in the corridors. Your personal scooter should be able to fit in a standard cabin with a limited entry doorway (58 cm on MSC Divina and MSC Armonia, 68 cm on MSC Lirica, MSC Sinfonia and MSC Opera, 62 cm on the others). If your scooter is larger than the dimension reported for your chosen ship, you should book an H cabin or rent a smaller scooter. Segways and other similar vehicles cannot be operated on board. They shall be confiscated and can be taken off the ship to be used while in port. Also please note that the aluminium gangways used if the ramp cannot be operated are less than 80 cm wide

Guests should provide a copy of a medical certificate confirming their existing pregnancy. Clinical cases include non-urgent general practice pathologies, adult and pediatric emergency medicine cases such as acute coronary syndromes, cardiac and respiratory failure, infective processes, anaphylaxis, trauma, orthopedic injuries and acute surgical conditions as well as occupational health issues and public health issues. Critically ill patients may require onboard medical treatment for several days in the medical center ICU’s which are equipped with telemetry, cardiac pacing, CPAP, electronic infusion devices, and mechanical ventilators.

Swing ride 160 feet above the water coming to MSC Cruises' new ship in 2025

MSC Cruises’ new ship will have a swing set on board – but not the kind you’d find on a playground.

Cliffhanger, a new ride launching on forthcoming U.S. flagship MSC World America next year, will allow guests to swing out over the ocean 160 feet above the water, the cruise line revealed Tuesday. The four-seat ride uses mechanical arms to lift guests beyond the 22-deck ship’s edge, where they are propelled back and forth.

The line is billing the attraction as the only over-water swing ride at sea. MSC Cruises USA President Rubén A. Rodríguez said Cliffhanger is a “natural next step,” following attractions on its other ships like Seascape’s Robotron .

“It’s the first time we’re taking thrill-seekers out past the deck and over the water, and we know they’ll love it,” he told USA TODAY in an email. There will be an added charge for the ride, but pricing details were not yet available.

Cliffhanger will be part of Family Aventura, one of seven districts on the 2,614-stateroom ship. The area also features The Harbor, an outdoor park area with a high ropes course, a playground inspired by the lighthouse at the line’s private Bahamas destination Ocean Cay MSC Marine Reserve and more.

Other districts on the ship include Galleria, home to dining venues, shopping and games “at the heart of the ship,” and Zen Area, an adults-only zone at the vessel’s stern where guests can enjoy music, catch some sun and take a dip in twin pools, according to an earlier news release .

Andrea Zito

Andrea Zito, Swan Hellenic’s Chief Executive Officer and a Reserve Officer in the Italian Navy, has a long and distinguished c-suite career in the cruise industry. Born in Trieste, he studied in Genoa, gaining an MSc in Naval Architecture and Marine Engineering. Remaining in Genoa, he worked in the offshore, shipping and cruise industries for many years before establishing himself in the Principality of Monaco.

His first job was with Saipem in Milano, where he worked in offshore vessels, semisubmersibles and jack ups. He then moved to a professional partnership with Martinoli Group where he was Project Manager for various Costa Cruises ships built in the early 1990 and managed several Costa vessels from Genova and Miami office before settling in Monaco in 1995.

When Costa was acquired by Carnival Corp in 1997, Andrea expanded the ship management activities in Monaco working for various operators of oil tanker operators to cruise companies, including Festival and Renaissance Cruises, until his company was acquired by V-Ships in 2005.

As Senior Vice President Operation Safety and Crewing, he managed Silversea’s new build programme and majority acquisition by Royal Caribbean. Then, in early 2020, Andrea embarked on another completely new adventure. Leaving Silversea, he and a small number of investors signed a contract for three new expedition ships from Helsinki Shipyard, while acquiring the Swan Hellenic name to renew its founders’ iconic vision of cultural expedition cruising.
