TRAVEL ADVICE

TRAVEL VACCINATIONS FOR PATIENTS AT CHATSWORTH ROAD MEDICAL CENTRE

If you are planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.  Vaccinations are available to protect you against infections such as Yellow Fever, Typhoid and Hepatitis A.  In the UK, the NHS routine immunisation schedule protects you against a number of diseases but does not cover all of the infectious disease found overseas.

When should I start thinking about the vaccines I may need?

You need to complete the "Travel Vaccination Advice" form and send this into us at least 6-8 weeks before you travel.  Some vaccinations need to be given well in advance to allow your body time to develop immunity.  Some vaccines involve several doses spread over several weeks or months.  If you have an incomplete vaccination history for Tetanus/Polio/Diphtheria, you may need extra doses (given a month apart). It is also now recommended that 2 x MMR vaccinations (given a month apart) are given to every adult travelling abroad (this does not need to be repeated after the 2 vaccinations).

You may be at more risk of some disease, for example, if you are:

  • Travelling in rural areas
  • Backpacking
  • Staying in hostels or camping
  • On a longer trip rather than a package holiday

If you have a pre-existing health problem, this may make you more at risk of infection or complications from a travel-related illness.

The vaccinations that are free on the NHS and are available at this surgery are:

  • Tetanus/Diphtheria/Polio
  • Hepatitis A
  • Cholera (very rarely required)

The vaccinations that would need to be assessed and given privately at a travel clinic, and you would have to pay for are:

  • Hepatitis B
  • Japanese Encephalitis
  • Tick-borne encephalitis
  • Tuberculosis (TB)
  • Yellow Fever
  • You will also need to be issued any Anti-Malarial medication via a travel clinic if needed

Things to consider when travelling abroad:

  • Mosquito/insect/tick bite repellents and prevention
  • Practice safe food and water hygiene
  • Ensure you have good travel health insurance
  • Long haul flights have a greater risk of Deep vein thrombosis, so leg exercises should be done and consider flight socks
  • If you are on medication, ensure you take this with you with proof of prescription

What to do next

  • Send in your Travel Vaccination Advice form (download HERE  or alternatively pick up a copy from reception and return to the surgery once complete) at least 6-8 weeks before travel.
  • The Practice Nurse will work out which vaccinations you are likely to need and complete the form, asking Admin to contact you.
  • Book an appointment with the Practice Nurse for any NHS, free vaccinations.
  • Make an appointment at a travel clinic if you need any private vaccinations.

Prescribing Sedatives for Fear of Flying

Chatsworth Road Medical Centre has taken the decision to NOT prescribe sedatives for fear of flying.  The decision has been made by the GP partners and this policy is adhered to by all prescribers working in the practice.  The reasons for this can be found below: 

1)  Diazepam is a sedative, which means it makes you sleepy and more relaxed.  If there is an emergency during the flight it may impair your ability to concentrate, follow instructions and react to the situation. This could have serious safety consequences for you and those around you.

2)  Sedative drugs can make you fall asleep, however when you do sleep it is an unnatural non-REM sleep. This means you won’t move around as much as during natural sleep.  This can cause you to be at increased risk of developing a blood clot (DVT) in the leg or even the lung. Blood clots are very dangerous and can even prove fatal. This risk is even greater if your flight is greater than four hours.

3)  Whilst most people find benzodiazepines like diazepam sedating, a small number have paradoxical agitation and in aggression. They can also cause disinhibition and lead you to behave in a way that you would not normally.  This could impact on your safety as well as that of other passengers and could also get you into trouble with the law.

4)  According to the prescribing guidelines doctors follow (BNF) Benzodiazepines are contraindicated (not allowed) in phobia. Your doctor is taking a significant legal risk by prescribing against these guidelines. They are only licensed short term for a crisis in generalised anxiety. If this is the case, you should be getting proper care and support for your mental health and not going on a flight.

5)  Diazepam and similar drugs are illegal in a number of countries.  They may be confiscated or you may find yourself in trouble with the police.

6)  Diazepam stays in your system for quite a while.  If your job requires you to submit to random drug testing you may fail this having taken diazepam.

We appreciate that fear of flying is very real and very frightening.  A much better approach is to tackle this properly with a Fear of Flying course run by the airlines and we have listed a number of these below.

Flight anxiety does not come under the remit of General Medical Services as defined in the GP contract and so we are not obliged to prescribe for this.  Patients who still wish to take Benzodiazepines for flight anxiety despite all of the above points are advised to consult with a private GP or travel clinic.

Travel Vaccinations

Why do you need to be vaccinated for travel.

Vaccines are mandatory for travel to particular destinations. For example, if you have passed through an area where yellow fever is known to occur, many countries won't allow you to enter unless you have been properly vaccinated. We are a Designated Yellow Fever Vaccination Centre. Our Travel Clinic Pharmacists can vaccinate and provide a certificate of proof or waiver.

Some places require you to carry an International Certificate of Vaccination to prove that you have been vaccinated against diseases prevalent in the territory.

Dr Nilu Vajpeyi

Dr Nilu Vajpeyi became the contract holder for Brigstock family Practice in March 2004. Since then she has worked alongside her husband Mr Christian Lyons who acted as the surgery’s business manager until August 2010 when he was also added to the contract. Dr Vajpeyi and Christian Lyons have managed to deliver improvements to the PMS surgery contract and other NHS services in Croydon by working as an affect partnership.

This partnership has delivered:

  • The first practice in Croydon to achieve a 2 year accreditation period for the IM&T DES
  • Remote home access to the surgery’s clinical system

Our Resources

Brigstock Family Practice runs from a freehold, substantial, modernised property, situated on Brigstock Road which is a busy main road in Thornton Heath. It is very conveniently located in the middle of its catchment area with easy access by public transport for patients and staff with plenty of unrestricted off-street parking for those who drive. The current practice list size is circa 4,700 and rising.

Dr Ravi Vajpeyi worked as a single-handed GP from this site from 1983 until her death in 2004. She was joined by her daughter, Dr Nilanjana Vajpeyi in October 2003 and the then practice manager, Christian Lyons, in December 2003. After Dr. Ravi Vajpeyi’s untimely death, Dr Nilanjana Vajpeyi signed the PMS contact with the then Croydon PCT on the 11th March 2004. 

In an effort improve the patient experience and the working environment of staff Dr Vajpeyi and Christian Lyons modernised the facilities in 2005 and 2006. This resulted in an increase in clinical space to the current 200m 2 . The refurbishment of the premises was designed by the award winning architect Parag Sharma. The refurbished premises now comply with DDA, fire, privacy, security, infection control and size requirements necessary to practice safe healthcare. The premises have been inspected by NHS Croydon, the Care Quality Commission and Oakleaf Surveyors. All have identified that the premises are well presented and fully fit for purpose.

What makes our work special.

Brigstock Family Practice strives to maintain the traditional family GP relationship valued by patients in a modern healthcare environment. We are a family practice and not a large medical centre or health centre. We feel that family GPs should know their patients and that an intimate professional relationship enhances the care they are able to provide. By taking this approach we aim to ensure that all care we provide is tailored to our patients’ specific needs. As well as providing bespoke patient focused care we aim to ensure this is delivered in a way that is consistent to the care we provide our private patients.

Modern practices now have many more responsibilities than traditional family GPs had in the past. These additional responsibilities have enhanced the services and that practices now provide and to help manage this Brigstock Family Practice has adopted many modern methods. This has necessitated the adoption of numerous computer hardware systems, databases and software applications. By embracing technology in this way we are better able to serve our patients in an efficient and organized way without jeopardising our traditional family GP approach.

Mayo Clinic Minute: What to know about Legionnaires’ disease

Legionnaires' disease  is a serious lung infection caused by Legionella bacteria. While most people exposed to the bacteria don't get sick, some people are at higher risk of infection and potentially serious illness. 

Dr. Nipunie Rajapakse,  a Mayo Clinic infectious disease expert, explains how this disease is spread and what you should know about Legionnaires' disease.

Legionnaires' disease is a type of serious pneumonia that people can get typically from exposure to mist or water. It's not spread person-to-person.

"Legionnaires' disease is spread through inhaling mist that has the Legionella bacteria in it," says Dr. Rajapakse.

"That mist can come from many different sources that use water. The more common places that we see outbreaks associated are whirlpools, air conditioning units or fountains," she says.

Home and car air conditioning units aren't a risk for Legionella growth because they don't use water to cool the air.

Not everyone exposed gets sick, but some people are more susceptible to illness than others.

"Certain people might be at more risk of getting Legionnaires' disease; we definitely see it more commonly in older individuals or people who have weakened immune systems," says Dr. Rajapakse.

It can take 2 to 4 days after exposure to feel symptoms.

"Symptoms of Legionnaires' disease can start as a flu-like illness with fever, body aches, cough, malaise, kind of feeling really tired; then [it] can present like pneumonia," she says.

It can progress to severe illness requiring hospitalization.

Although the infection primarily affects the lungs, occasionally, it can cause infections in wounds and in other parts of the body, including the heart.

"When identified and treated early, it is treatable with antibiotics," says Dr. Rajapakse.

Iowa clinics rushed to treat women before abortion ban began. Some couldn't make it in time

Abortion clinics across Iowa say they plan to continue operations after the state's six-week abortion ban officially goes into effect Monday morning ― even as their ability to provide abortion care will be drastically curtailed.

Iowa's so-called "fetal heartbeat ban" is enforceable as of 8 a.m. Monday, prohibiting abortion once fetal cardiac activity can be detected by ultrasound, typically around six weeks of pregnancy. The only exceptions are cases of rape, incest, fatal fetal abnormalities and to save the life of the pregnant woman.

The ban is expected to block nearly all abortions in the state, with Planned Parenthood North Central officials estimating the number of procedures performed in the state could drop by at least 97%. More than 4,000 surgical and medication abortions were performed in Iowa in 2023, according to estimates from the Guttmacher Institute .

Last week, abortion clinics across the state worked to see as many patients as they could before Iowa's ban went into effect. Alex Sharp, senior health center manager for Iowa's Planned Parenthood clinics, said the clinics overbooked appointments with patients who were originally schedule to be seen for care this week, after the new ban is in place.

However, clinic staff came across many patients were were unaware of the pending ban, Sharp said. She estimates 30% of the patients weren't able to come to an earlier appointment because they couldn't get time off work or find child care on short notice.

“I would say we were able to get in maybe 50% of the people that were scheduled for next week, but for the rest of them, the fact is that it was too late of notice for them to rearrange their schedules to be able to come sooner,” Sharp said Friday.

Iowa abortion clinics will continue abortion care under strict law

The new six-week law was passed in a 2023 special session called by Republican Gov. Kim Reynolds, but it was blocked from taking effect by a Polk County District Court judge.

The state appealed to the Iowa Supreme Court, which issued a ruling June 28 ordering the district court to lift its injunction and allow the law to take effect.

Reynolds, who has led the effort to enact the six-week ban since 2018, called the Iowa Supreme Court decision last week "a victory for life."

“There is nothing more sacred and no cause more worthy than protecting innocent unborn lives," Reynolds said in a statement.

Pulse Life Advocates Executive Director Maggie DeWitte applauded the court's decision to lift the injunction, saying in a statement last week that abortion opponents in the state celebrate the victory "with great joy and gratitude and abundant blessings."

"Monday is an important step forward for the protection of life in Iowa, but we will continue to work toward our gold standard of life at conception," DeWitte said. "We will continue to fight for the lives of all of our preborn brothers and sisters until the day they are all protected under our law."

Abortion care providers have strongly denounced the 6-week ban, saying the negative effect on Iowans will be "widespread and generational."

“We now live in a reality where politicians have control over Iowans' bodies and futures,” Dr. Sarah Traxler, chief medical officer of Planned Parenthood North Central States, said in a media briefing Friday.

Planned Parenthood and the Emma Goldman Clinic in Iowa City, the state's other surgical abortion provider, say they intend to continue offering abortion services within the parameters of the new law.

Moving forward, Traxler said Planned Parenthood clinics will continue to schedule patients for abortion care in Iowa if their pregnancy has not yet reached six weeks of pregnancy, or two weeks after their first missed menstrual period.

If patients are unsure of their last menstrual period, they are given the option to still be seen in Iowa or be scheduled for an appointment out of state.

Providers and patients also will continue to grapple with challenges arising from a law mandating a 24-hour waiting period for an abortion in Iowa. That law went into effect in 2022. Traxler said the law adds additional time before a patient can access care, creating inconvenience for patients as well as clinic operations.

Even if patients qualify for abortions under the new law's exceptions, some have speculated a lack of clarity from the Iowa Board of Medicine, the enforcing agency of the law, could mean patients will be turned away from care.

The administrative rules from the board offer no insight on the penalties doctors could face under the law, but the potential risk of losing their medical license could mean doctors will hesitate to act, opponents say.

"This ban ties doctors’ hands. It puts them in the impossible position of deciding whether to keep their license or provide life-saving health care and uphold their oath," Traxler said. "Every person comes to us with different and unique circumstances. They want full information about health care options that are best for their lives and their futures, and they expect to receive the care that they want and need. That unfortunately stops on Monday."

Advocates boost efforts to help patients travel for abortion care

Abortion care providers and other reproductive rights advocacy groups anticipate many Iowans will be forced to seek care out of state, and have worked to boost resources to help those patients reach their appointments.

Ruth Richardson, president and CEO of Planned Parenthood North Central States, said the organization has expanded capacity at clinics near the Iowa border, including its clinic in Mankato, Minnesota. Richardson said an expansion at the Omaha clinic is underway and will quadruple the number of exam rooms from four to 16.

However, Traxler said the Mankato location offers only medication abortion, meaning patients seeking a surgical procedure must travel to the Twin Cities.

Already, groups in nearby states without abortion bans are seeing an influx of Iowa patients. The Chicago Abortion Fund, which recently partnered with the Iowa Abortion Access Fund in light of the pending ban, says it's seeing the highest call volume to its hotline in its 40-year history.

In the first three weeks of July, the Chicago Abortion Fund saw 60 requests for support from Iowans anticipating the ban, a 165% increase compared with previous months, according to data from the Iowa Abortion Access Fund.

The state's six-week ban will dramatically shift the legal landscape for abortion not just in Iowa, but across the Midwest. With this new law, Iowa has joined 17 states ― mostly in the South ― that ban abortion entirely or as early as six weeks of pregnancy, according to an analysis of nationwide bans .

Since the U.S. Supreme Court's Dobbs decision in 2022, Iowa abortion clinics have seen many patients traveling from Missouri, which enacted a total ban on abortion, Traxler said. Most out-of-state patients in recent months have come from Nebraska after a state law there implemented a 12-week ban on the procedure.

Illinois and Minnesota are two neighboring states where most Iowans are likely to travel, but already, clinics there have seen a huge surge in out-of-state patient volume. The Guttmacher Institute estimates the portion of out-of-state patients to both states has more than doubled from 2020 to 2023.

Richardson said Planned Parenthood North Central States' patient navigators have helped 4,000 patients access care at an out-of-state clinic since 2022. Still, the possible strain on these clinics adds to the growing concern of abortion rights advocates in Iowa who worry about patients' ability to access abortion care in the future.

Already, patients lack the resources ― including financial ― to travel for care. Kristina Remus, a patient services associate with Planned Parenthood North Central States, estimates only about half of the patients she's worked with in recent weeks have told her they have the resources to be able to travel for care.

"Abortion bans do not ban abortion for everyone. They ban abortion for people who do not have the means or the opportunity to travel," Richardson said.

Abortion rights advocates have also raised concerns about the negative consequences for the health of Iowans. They point to studies that show infant mortality rose in states with a total ban, or reports that maternal deaths are more likely to occur in states with laws restricting abortion.

Iowa already has poor maternal health access, with one-third of its 99 counties designated as maternal health deserts, meaning they lack access to OB-GYNs or a birthing hospital, according to the March of Dimes .

Even as abortion clinics have prepared logistically for the ban, the reality of its impact on patients' health and well-being is a bitter pill for clinic providers and staff, they say.

“Come Monday, abortion access will look very different in Iowa," Sharp said. "It's a devastating blow, and it's hard to swallow.”
